Dementia Care –
Nurseplus Homecare Services support people with dementia to live well at home for longer, we identify practical support to the presenting needs. We acknowledge and support the change in requirements as dementia progresses, this is crucial to minimising stress and where possible reduce the risk of potential need for care home placement.
We provide our care staff with specific ongoing training for dementia needs, this includes facts about dementia, knowledge about the different types/stages of dementia and skills to respond to a range of issues that might arise and recognising when specialist intervention is required.
Complex Care & Palliative/Endstage -
Clients receiving complex care have substantial and ongoing healthcare needs. Individuals who require or are in receipt of complex care can be the result of chronic illness, disabilities or following hospital treatment. Complex care is sometimes referred to as long-term care on continuing care.
We understand that living with a long-term condition and struggling from lack of independence can be very challenging. Our staff provide tailored support to individuals and their families with complex care needs the support our clients need to manage their condition and maintain the lifestyle they choose.
Nurseplus have various experience of services to support Service Users with complex mental health, physical or learning disabilities needs. With our main aim to maintain a safe place for them to live, whilst enabling them to recover, sustain recovery, be socially included and to gain, regain or maintain independent living skills.
Children and Young People Support Services -
At Nurseplus we support children/young people to increase and develop their independence by providing the tools to develop their own skills. We support the young person to experience new things, particularly things which may provide useful skills.
In order to ensure we are always working towards achieving outcomes, we would present their individual goals in a way they would find easy to understand.
• We would break the outcome down into smaller aims.
• For example, we may use pictures and charts to demonstrate and log progress.
• As the physical needs of the child/young person can change and be dependent on many contributing factors, Nurse Plus understands the importance of having outcome based care.
Care Package example:
08:00 - 08:30 - 30 minute call - To assist with personal care needs, assist with breakfast
12:00 - 13:30 - 1.5 hour call -Take food shopping, prepare lunch, light household duties, assist with toileting
20:30 - 21:00 - 30 minute call - To assist with personal care and going to bed
Client Goals include:
